VFC Patient Breakdown (Washington)
This report counts the number of patients that received a vaccination during the specified date range. The counts are separated by age ranges of <1, 1, 2, 3-5, 6, 7-10, 11-12, and 13-18 at the top of the report and by VFC status on the side of the report. Totals are calculated for each age group and VFC status. If the report is limited by County, this is the county of the Organization (IRMS) location, not the patient's county of residence.
Run the Report
To run this report, click the Reports > State Reports > VFC Patient Breakdown link. The VFC Patient Breakdown page opens. Â Enter the criteria and click Create Report. The report displays in a separate browser tab and can be printed using the browser's print function.
The available report criteria fields are as follows:

Person Completing Report |
Enter the name of the person completing the report. |

Phone Number |
Enter the person's phone number. |

Organization (IRMS) / Organization (IRMS) Group / Do Not Limit |
To limit the report by an Organization (IRMS) or Group, select it from the related drop-down list, after which the radio button is automatically selected. Do Not Limit is the default. |

Facility / Facility Group / Do Not Limit |
To limit the report to a specific Facility or Facility Group, select it from the related drop-down list, after which the radio button is automatically selected. Do Not Limit is the default. |

VFC PIN |
To limit the report to a specific VFC PIN, select it from the drop-down list (after which the radio button is automatically selected). |

Provider County |
To limit the report to a specific provider county, select it from the drop-down list, after which the checkbox is automatically selected. |
|
Vaccination Date Range |
To limit the report to a specific vaccination date range, enter the From and To dates, after which the checkbox is automatically selected. |
