This report creates a Washington State School Immunization Certificate that validates student/patient immunization records against Washington State School or Child Care immunization requirements. Search for the patient before running this report. This report can also be run directly from the Reports > School Reports page and from the Patient Demographics page if the state-configurable option has been selected. This report can also be scheduled to run from the Reports > State Reports page.
The Certificate of Immunization Status (CIS) displays the immunizations, dose number, date of immunization including HPV, and Varicella information for a specific patient. HepB doses display together excluding HepB 2 dose, which displays with the HEPB-2 label. The address, patient/guardian name, and phone number do not appear on the form.
When a patient has a history of single antigen dates for measles, mumps and rubella, six different dates, the logic is as follows:
Note that vaccines that are administered as confidential remain confidential and do not appear on the form.
To run this report, search for the patient and then click the Reports > State Reports > Patient Specific > Certificate of Immunization Status (CIS) link. The Certificate of Immunization Status (CIS) page opens. Select the Certificate Type from the drop-down list (i.e., Child Care, School Nurse) and then the Series from that drop-down list, then click Create PDF. The PDF version of that certificate for the selected patient automatically downloads.
