Adult Immunization Record (Tennessee)

The Adult Immunization Record is a pre-populated college health record (in PDF format) containing State of Tennessee mandated, as well as Advisory Committee on Immunization Practices (ACIP) adult recommended, vaccination statuses for matriculating college students. The record includes a forecast for due and past due vaccines. The record is intended for use by college administrators or clinicians to verify student compliance with vaccines recommended for college-age adults.

To run an Adult Immunization Record, the user must first select at least an Organization (IRMS) and then search for a patient. Click on the patient's name in the search results lists, which opens the Patient Demographic Master View page. Click on the Patient Specific Reports section to open it, then click the Adult Immunization Record link.  

After a patient is selected, this report is also available from Reports > State Reports in the menu.

The Adult Immunization Record for the patient appears pre-populated in PDF format and can be printed and/or downloaded.
