Child Care Module (Arizona)

Arizona Child Care module users can enter vaccine exemptions on the patient demographic screen.

To enter an exemption, first search for the patient. After locating the patient, click the Exemptions button.

In the Add Exemption section, select the Facility, Vaccine, and Exemption reason from the drop-down list and enter the Expiration Date. If this is a permanent exemption, select the Permanent option. Optionally, in the Additional Disease Information section, enter the month/year and age. Click Add Exemptions when finished.
